Pre-anesthetic anxiety is a common problem among patients undergoing cesarean section and may be accompanied by physiological responses such as increased blood pressure and heart rate. This community service project aims to reduce pre-anesthetic anxiety through education and the implementation of guided imagery therapy for cesarean section patients at Fatimah Islamic Hospital in Cilacap. The activity involved 35 participants and was conducted from May 26 to June 8, 2026. The stages of the activity included participant identification; education regarding the surgical procedure, anesthesia, and guided imagery; anxiety assessment using the Visual Facial Anxiety Scale (VFAS); the implementation of guided imagery using an audio guide for 5–7 minutes; and a follow-up assessment. Data were analyzed descriptively using frequency distributions, percentages, and the mean difference in anxiety scores. Before the intervention, 19 participants (54.3%) experienced severe anxiety, 14 participants (40.0%) experienced moderate anxiety, and 2 participants (5.7%) experienced mild anxiety. After the intervention, no participants had severe anxiety; 18 participants (51.4%) had moderate anxiety, 14 participants (40.0%) had mild anxiety, and 3 participants (8.6%) had no anxiety. The average decrease in anxiety scores was 3.40 ± 0.736 points. Physiological responses also showed improvement: 27 participants (77.1%) had normal blood pressure after the intervention, and all participants (100%) had a normal heart rate. The results of this study indicate that education combined with guided imagery can be used as a simple, safe, and practical nonpharmacological intervention to help reduce pre-anesthesia anxiety in patients undergoing cesarean section.
